We're looking for a detail-oriented Medical Records Specialist to join our office team and help ensure the accuracy, organization, and compliance of our clinical records. This is a full-time, onsite position based in our San Diego office.

Position Summary

The Medical Records Specialist is responsible for managing incoming and outgoing medical records and supporting communication with referral sources, physicians, and internal departments. This role plays a critical part in maintaining accurate documentation, ensuring regulatory compliance, and supporting efficient agency operations.

Key Responsibilities

  • Process daily clinical record functions in a timely and accurate manner.
  • Maintain and organize patient medical records in accordance with agency policies and regulatory requirements.
  • Work closely with Billing, Accounting, Clinical, and Operations teams to obtain required documentation.
  • Follow up with physicians, referral sources, and healthcare providers regarding missing or incomplete documentation.
  • Monitor outstanding documentation and escalate concerns to leadership when appropriate.
  • Ensure compliance with all applicable federal, state, and agency medical record requirements.
  • Assist with document management, filing, scanning, and electronic record maintenance.
  • Maintain confidentiality and safeguard protected health information (HIPAA).
  • Build and maintain professional relationships with physicians, referral sources, and internal team members.
  • Support the Manager/Administrator with operational needs and communicate concerns that may impact workflow or compliance.

Qualifications

  • Minimum of one (1) year of administrative experience in a professional office environment; healthcare experience preferred.
  • Previous home health or medical records experience is strongly preferred.
  • Typing proficiency of at least 45 words per minute.
  • Proficiency with Microsoft Office and electronic document management systems.
  • Strong organizational skills with exceptional attention to detail.
  • Ability to prioritize multiple tasks in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Knowledge of medical terminology and healthcare documentation preferred.
  • Strong customer service mindset with the ability to work collaboratively across departments.
  • Ability to maintain confidentiality and handle sensitive information professionally.
